Intense Pulsed Light (IPL) therapy has been explored as an emerging treatment for dry eye syndrome, particularly for patients with meibomian gland dysfunction (MGD), which is a common underlying cause of evaporative dry eye. IPL therapy works by targeting the dysfunctional meibomian glands in the eyelids, improving their function and subsequently relieving dry eye symptoms.
Mechanism:
The primary goal of IPL therapy for dry eye is to address MGD, which involves blockage or dysfunction of the meibomian glands in the eyelids. These glands produce the oily component of tears, which is crucial for preventing the evaporation of tears and maintaining a stable tear film. When these glands become blocked or don't function properly, it can lead to decreased tear quality, excessive tear evaporation, and dry eye symptoms.
IPL therapy works through a process called photothermolysis. It uses pulses of intense light to target and heat the blocked or dysfunctional meibomian glands. The heat generated by the IPL treatment helps to:

Melt Meibomian Gland Blockages: The heat softens and melts the waxy, solidified oils within the blocked meibomian glands, allowing them to flow more freely.

Procedure:

Diagnosis: Before the treatment, your eye specialist will thoroughly examine your eyes and evaluate the severity of your dry eye condition. Meibography and Lipid Layer Thickness are also evaluated.

Preparation: The patient is required to wipe their make-up off and wash their face.

Protection: To protect your eyes from the intense light, your specialist will apply protective corneal shields.

Application of Gel: A cooling gel or gel-like substance is applied to the skin around your eyes to enhance the light penetration and protect the surface of your skin.

IPL Treatment: The IPL device emits pulses of intense light that are directed at the skin of the lower eyelids, the upper eyelids, and sometimes the upper cheeks as well. The light energy is absorbed by the meibomian glands, generating controlled heat.

Post-Treatment Care: After the procedure, you may experience some redness or warmth in the treated area, but this typically subsides quickly. Your vision can be blurry due to the applied ointment. You can resume your regular activities immediately following the treatment. Sun exposure should be limited for 1 week. Other cosmetic procedures is advised to be refrained for up to a month.
